Medical Forms

The following forms are provided for the use of medication in school. No medication can be issued in school without the appropriate form. Please contact the school if you are unsure which form to use or if you need a hard copy.

Form 1 – Non-prescribed-medication

Use Form 1 for non-prescribed medication to be administered by the school.

Form 2 – Short-term-prescribed

Use Form 2 for short term prescribed medication to be administered by the school.

Form 3 – Long-term-prescribed

Use Form 3 for long term prescribed medication to be administered by the school.

Form 4 – Long-term-as-required-prescribed

Use Form 4 for medication that will be taken/administered as required, e.g. asthma inhalers.

Form 5 – Self Admin

Use Form 5 for medication which should be self-administered.
